Fourth Monkey Media Consent Form

Fourth Monkey Actor Training Company (Fourth Monkey) would like to explore ideas, take photographs, film and/ or audio recordings of the workshop/ event that you and/ or your child is taking part in. We will only use the ideas, photographs, film and/ or audio recordings if we have permission to do so.

2. Purpose

The purpose of collecting and processing your personal data (in the form of photos, videos, and audio recordings) is for the following legitimate interests. This includes, but is not limited to:

Promotional and Marketing Material: To create marketing material for Fourth Monkey and our partners, which includes the creation of print, brochures, posters, digital newsletters, and other promotional materials to attract future students and showcase the quality of our training.
Creative Content: For the creation of new materials for our open events, workshops, and actor training courses. This may include using the recordings in their original form, or in altered forms with appropriate credit.
Website and Social Media: To be used on our Fourth Monkey website and social media channels (e.g., Instagram, Facebook, TikTok, YouTube, X) to celebrate student achievements, share performances, and engage with the wider community.
Publicity: For distribution to news media outlets to promote University events and activities.
Educational and Archival Use: For internal use, such as documenting progress, archiving performances, and for staff training purposes.
Funding and Evaluation: To provide evidence and reports to funders and partners of our work and its impact, and for internal evaluation of our programs.

This form should be signed by the participant and/ or parent/ carer of a child or young person under the age of 18; doing so provides permission for the photographs, film and/ or audio recordings of the child/ young

person to be used by Fourth Monkey as outlined above. Please note that you will have to fill in a separate form for each child.

I, the named parent/ carer, consent to Fourth Monkey Actor Training Company developing ideas photographing, filming and/ or audio recording the involvement of the named participant in its activities under the stated conditions. I confirm that I am legally entitled to give this consent. I also confirm that the named participant is not under a court order.

I do not consent for my photo to be used on this occasion
